一、哮喘 Asthma

某女60岁， 初诊2014.2.9    面诊

 女儿泣告：母亲喘不能步。西医弃治。一周前从老家接来青。

路上着凉，开始喘、夜重。从卧室走到厕所也会致喘。可明显听到曳锯声、似乎里面有痰，但自称无痰。觉胸腔热、喜冷饮。喘甚，夜不得卧睡，倚靠沙发而睡。夜尿2次。右寸未觉浮数。双尺未触及。神可。

印象老慢支新发哮喘。

2002年脑血栓梗阻，右侧偏瘫。西医治疗，肢体见好转，但至今右下肢痿躄不动。语涩、流涎、面黄浮肿、舌长过唇。

印象：风寒束表、肺郁发热。似非麻杏石甘汤证，因双寸关均平，无右脉症。

治则：降气、化痰、平喘，解表闭，但因渴甚且喜冷饮，酌清里热，肢冷加温阳药

鉴于身体太虚弱，未予针灸。但用方药。

一剂后，可高枕而睡，二剂后可正常卧睡。二诊已见在室内推车散步。已无苦状。

  2016.1．8     网诊

电告：外甥得病毒性感冒住院。宿主在家无人照顾，着凉，发喘不得息，西药不能控制，亦住院。医用氧化钠、克林霉素酸脂、氨溴索注射液、丙茶碱注射液、地塞米松磷酸注射液、头孢呋辛钠，心电图心衰，窦性心动过速，陈旧性心肌梗塞。T波异常。4天抗生素治疗未得到缓解，昨天稍缓，医院劝其出院。自23号开始已经又不能吃饭了，喉间痰鸣甚，痰盛，喉咙里有痰咳不出来。白色泡沫状痰。唇面、四肢发青。她现在手指发红指甲呈粉红色: 早晨血压100/143，心跳96。

印象：仍是外感触发哮喘、表闭，热郁于胸膈不得发散，痰气盛而不降，肾气弱，无力咳出。外感住院西药治疗，伤阳更甚，寒邪内伏太少二经

治则：发汗解表、化痰降气

二剂药后喘平痰消，开始进食，吃饭不呛了，一次可吃一小碗面条。但睡觉黑白颠倒。大便三天一次，结秘之状减轻。此乃体虚，阴阳俱若甚，须长期调养。

 小结：哮喘病发多因外感，治当辨清病因，循证而治。如有表闭，当解表为要，汗法当为首选。若但见喘息，一味平喘降气，不仅徒耗肺气，且表寒内陷，易发他证。若证见少阴病证，为太少同病，又当酌用肾药，鼓舞肾气，助透邪出表。但病家常因病缓无苦便不再治疗，或有它因，因此不易除根，一旦着凉，势必再发，实为憾事。



Asthma

Daughter stated complaint for mom.

Severe Ashma, Gasp for breath only a few steps' walk. Hospitalized but symptoms were not control, so discharged.

On the way from hometown to Qingdao, recaught a cold. breathed even more heavily.

could not lie down to sleep, lean against back of recliner to sleep. prefer drinking cold water. sensation of heat in chest. sounded strong phlegm when breathing, but unable to spit out.

Right side limps paralyzed due to cerebral thrombosis. Movement improved after WM treatment. Lower leg still could not move. uncontrollable saliva, tongue over reached lower lip, felt difficult to talk.

Impression: external coldness and stagnated heat in Lungs.

Principles: descend Qi, ease gasp, remove phlegm.

No acupuncture conducted due to too weak body condition

After taking one doze, sleep lying with high pillow. After two doses, sleep lying like normal.

2nd home visit. the patient was walking in waiting room with a walker without any breathing problem.

Similar symptoms recurred after catching a cold. treated the similar herbal therapy and back to normal in three days.

WM's treatment of Asthma is focused on "Not Gasp", while CM focus on the recovery of lungs function. Many CM doctors claimed Asthma is absolutely curable. I could not agree more.

 Salbutamol metered dose inhaler is no more than a placebo decrease symptoms which is good. But the disease is still there. Herbal therapy would be the only effective treatment, totally eliminate the symptoms. Let you enjoy the long-forgotten joy of free breath.
