六、前列腺炎 Prostatitis

某男，58歲  初诊 2014.8

小便多年不利，感冒發燒後愈甚，嚴重時癃閉，點滴不出。近日小便短少、解便小腹部發熱、酸脹，一日數十次小便，且每次不超過50cc。夜裏小腹脹痛，不能尿出，直到放屁以後，才可解出。肛門亦有不適。前列腺部位時有隱痛。憋尿時間僅時數分鐘。

2003年2月、2009年12月，筆者曾2次治療過該患者急性尿瀦留，治愈後沒有後遺癥。

前列腺炎是個頑癥，因為這個部位一是藥物不容易滲入，另外，本為痰血瘀阻，一旦受到刺激，比如憋尿、性事，便容易充血，造成新的瘀滯。病情稍有緩解，原來的性功能就可能恢復，一有性刺激，立刻復發。但總歸痰血瘀阻於陰器。肝經環繞陰器，所以治療此病不能不考慮肝的問題。另外，此病容易和膀胱炎混淆。膀胱病變可能有小便不利，可能屬於上焦氣阻，當治以宣肺降氣，也可能膀胱蓄血證，治以活血化瘀；也可能是膀胱蓄水，當行氣泄水。還可能與尿道炎混淆，但彼多尿痛甚，有上焦心火，此則很少與心火有關的癥狀。

病機：下焦痰血瘀阻，肝氣郁結

治則：行肝軟堅、化痰軟堅，治以削法

此病針灸有效，只是部位隱諱，有的患者不願嘗試。該患者配合很好，凡有效的治法都能接受。

針、藥後2周，尿流開始變粗，酸脹感減輕，待尿時間大為縮短，尿前矢氣減輕，憋尿時間大大延長，可達數小時。一個月後，尿流接近正常的75%，小便已無不適。2014年底因沒有癥狀，放松警惕，性事頻繁，再次出現上述癥狀。幸好早有準備，適時而止，未釀成大病。

此次更有膽結石證一起出現，右肋下痛，B超查有1.9x1.2結石，B超顯示前列腺硬化，有B超檔。這正是中醫的治域。基本延續原來治則，更以軟堅消磨之劑，癥狀再次明顯緩解，45天後，膽結石已經化為2小塊，合並體積縮小約25%。  小便接近正常的70%，憋尿後，小便待尿時間縮短超過70%。後因外出，用藥不便，暫停醫治。

筆者認為，前列腺炎並非如西醫所說不可治愈，但卻屬難治之癥。但只要辯證得法，持之以恒，還是可以做到臨床治愈。但難的是色為天性，比固習更難以遏制。此病多犯好色之人，此為證。

Difficult urination for years, especially during fever which might cause retention of urine, twice completely urination blocked, in Feb 2003 and Dec 2009, when I treated with herbal therapy. Current symptoms: difficult urination, feel burn at lower abdomen, severe distention, urinate oven a dozen times, each time no more than 50cc. At night, only more difficult urination, however after gas release, the urinate would come out. Anus and Prostate uncomfortable.

Impression: depression of liver Qi with phlegm and heat

Herbal therapy and acupuncture

Sensitive point. Thanks to the cooperation of the patient, I could do needles on perineum besides other point. About two weeks later, from dribbling to stream, time of holding urinate last much longer. A month later reached about 75% quantity of urination. Around end of 2014, due to decreased symptoms, the patient had sexual activities that triggered a severe distention of urination again. Repeated treatment. Luckily enough no more danger
