十一、再障贫血 MDS

初診：2013.10.31,   男，60 歲，

病史：膽囊切除。有入住新房氣味異常的經歷。

刻診：血小板WBC 6000 。 脈沈、數、大，關脈尤甚。面色晦暗、發白。雙腳腫大，不能穿鞋。說話聲很低，腳腫甚，心腎衰竭之兆。病人此前曾在他處服用中藥，未能得到完全控制，喪失信心，隨轉入西醫治療，經治數年，其中近一年服用西班牙出產生血小板藥（每粒約2000元）最後出現是狀。每早10點仍不願起床。疲倦甚。說西藥無用，想轉用中藥，雲“中藥不可能比西藥更壞吧”。

病機：肝腎陰虛，久則暗耗陰血，膽囊切除，造成肝無法排毒經膽囊出體外。

治則：清理肝膽（雖切除，治法不變）、排毒消風、潤肝補血、滋腎填髓

2013.11.1 藥進一付，第二天早上7點起床，在陽臺上打電話。自稱精神很好，腳已經消腫。感到很不可思議，於是決定再去醫院查血。抽血後返回。下午，醫院來電話，告“你血小板沒了，馬上回來住院”。病人及家屬大驚，立刻乘車去醫院。途中，醫院來電話，稱剛才是實習醫生，沒看懂驗血報告。血小板17，以為就是沒了。應為17000（通常後的三個0不寫）。於是，車到醫院，沒有進門又返回。病人更加不解，怎麽會一副藥血小板翻2倍？決定第三天又去醫院檢查，結果血小板18000，始才相信。可惜的是，病人一直堅持西醫為治，中醫為輔。最後決定骨髓移植。術後血小板在低水平徘徊，堅持了一年。一年後再次做骨髓移植。第二次只堅持了幾個月，很快進入病危狀態，最後不幸過世。十分遺憾。

值得一提的是，2010年，患者曾在北京花2000元掛號費，經名醫張XX處方：紅小豆50斤。筆者得知後立告：禁服此方。其稱紅豆補血。豈不知大食紅豆，其滲瀝之性足以耗幹津液 --- 造血最寶貴的原料，血萬無再生之理。此公知其一，不知其二，幾釀大禍（這也是所謂“中醫”引起爭議之處：妄稱神醫，誤人害己）。此病，西醫多使用激素治療，取效一時，預後不良。譬如擰毛巾得水，再擰再得水，一次比一次力氣大，最後毛巾裏沒有水了，力氣再大也無用，也就無計可施了，因為根本不是造水。中醫則有明顯的優勢。除治不離肝腎外，還應結合病患具體情況，復方結合，僅靠單方、偏方難以湊效。此病不是一朝一夕所得，便不是一朝一夕可愈，須堅持服藥，最後一般可以得到完全緩解，並極有可能治愈。但因於體質差異，即使臨床治愈，亦需謹慎，不可大意。

60-year-old male， Oct 31, 2013

History: Gallbladder removed. Moved to new house year ago and sniffed bad smell.

Current: PLT (blood platelet) 6x10³, according to the doctor, his PLT too lower to take BMT (bone marrow transplantation)  . Greyish pale look, swollen feet and could not wear sandals. Talked in very low voice when talking. Take herbal therapy in the past few years and failed to get improvement and turn to WM. Taking drugs made in Spain to generate （about $300 per pill) PLT. fatigue, would not get up till 10:00am. Saying drugs were useless and would like to try herbs again.

Impression: Yin deficiency in Liver and Kidney and toxify in Liver. Both Heart and Kidney failure.

Principle: detoxicate and tonify Qin

Nov 1, after taking one dose the night before, got up at 7:00am and seen talking in loud voice over a phone call at the balcony. Swollen on feet were gone completely. The patient felt unbelievable the effectiveness of the herbs was, only one dose. He was determined to check the blood again. the lab result came in the afternoon: 17x10³. Almost tripled? NO way. Again, he took a blood test the third day. the result: 18x10³. Now he believed it was the herbs because in the past seven years, he never had the PLT increased so dramatically. He was happy because the doctor told him he could take BMT (bone marrow transplantation) when his PLT was above 20x10³. In spite of the fact I told him the side effect of BMT. He insisted because the marrow donner has the perfect matching points ten with him, a very rare case that he could not resist.

The patient made a wrong choice. After the first BMT, everything seemed ok for about one year. Then the 2nd BMT. He only survived a few months.

I would not say I could have saved him. But most likely he would not die in such a short time if he took herbs.

When practicing CM in the past few years treating blood diseases or cancers, I had a very weird finding: richer people die much earlier than poor people. The former usually can afford high-level treatment, such as BMT, and the poor who can only afford herbal therapies would survive much longer than the richer, if not cured.

This case reminded me of my teacher said," in practice you will find diseases are often less difficult things to deal with than the patients. "

In the past few years, I advocated for using herbal therapies to deal with the blood disease by writing to the parents of the patients, two in the U.S, four in China, I offered complete free herbal therapy though. None of them took my advice and none of them survived longer than one year. Most of them were kinds or young adults. I felt so sorry for the young life because I believe all the denial were made by others, not themselves. They were too weak and too young to make a decision but had to undertake the results.
